1. About NeuroKare

NeuroKare is a medical clinic providing physician-guided telemedicine services through the NeuroReset Program, operated under the supervision of Dr. John Lee, M.D., a dual board-certified physician. Our services are currently available to residents of states where Dr. Lee is licensed to practice medicine.

2. Medical Disclaimer

The NeuroReset Program is a physician-guided medical program. However, the information provided on this website is for general informational purposes only and does not constitute medical advice.

• Results vary from patient to patient and are not guaranteed
• The program is not a substitute for emergency medical care
• Low Dose Naltrexone (LDN) is prescribed only when clinically appropriate, at the sole discretion of Dr. Lee
• Purchasing a program tier does not guarantee a prescription will be issued
• If you are experiencing a medical emergency, call 911 immediately

By purchasing the NeuroReset Program, you acknowledge that you have disclosed all relevant medical history to Dr. Lee and that you are participating voluntarily.

3. Eligibility

To use our services, you must:

• Be 18 years of age or older
• Be located in a state where Dr. Lee is licensed to practice medicine at the time of your consultation
• Provide accurate and complete health information during your evaluation
• Not be in an acute medical emergency requiring immediate in-person care

NeuroKare reserves the right to decline service to any individual who does not meet eligibility requirements, including patients located outside of licensed service areas. In such cases, a full refund will be issued.

4. Program Tiers & Services

The Essential NeuroReset — $299

Includes one in-depth video consultation with Dr. Lee, one follow-up appointment, standard scheduling, one month of access to the NeuroReset AI Assistant, and a one-month LDN prescription if clinically appropriate.

The Signature NeuroReset — $599

Includes one in-depth video consultation with Dr. Lee, two follow-up appointments, standard scheduling, three months of access to the NeuroReset AI Assistant, and a three-month LDN prescription if clinically appropriate.

The Elite NeuroReset — $1,299

Includes one in-depth video consultation with Dr. Lee, six follow-up appointments (bi-weekly), priority scheduling, complete access to the NeuroReset AI Assistant, and a three-month LDN prescription if clinically appropriate.

All program tiers include a comprehensive evaluation to assess candidacy. Purchasing a program tier does not guarantee any specific clinical outcome or that LDN will be prescribed.

5. Payments & Refunds

All payments are processed securely through Square. By completing a purchase, you agree to Square's Terms of Service in addition to these Terms & Conditions.

Refund Policy

• Full refund: If you are deemed ineligible following your initial evaluation (e.g., outside licensed service area, medical contraindication), you will receive a full refund
• Pre-consultation cancellation: Cancellations made more than 48 hours before your scheduled consultation are eligible for a full refund minus a $25 administrative fee
• Post-consultation: No refunds will be issued after your initial consultation has been completed
• No-show policy: Missed appointments without 24-hour notice may be forfeited without refund

6. Telehealth Consent

By scheduling a telemedicine consultation with Dr. Lee, you consent to the delivery of healthcare services via electronic communications, including video conferencing. You understand that:

• Telemedicine has limitations compared to in-person care
• Technical difficulties may occasionally affect the quality of your consultation
• Your health information will be transmitted electronically and protected in accordance with our Privacy Policy
• You may withdraw consent for telemedicine at any time by contacting us

7. Intellectual Property

All content on this website and within the NeuroReset Program — including text, graphics, logos, program materials, and the NeuroReset AI Assistant — is the intellectual property of NeuroKare and is protected by applicable copyright and trademark laws.

You may not reproduce, distribute, modify, or create derivative works from any NeuroKare content without our express written permission.

8. Limitation of Liability

To the fullest extent permitted by law, NeuroKare and Dr. John Lee shall not be liable for any indirect, incidental, special, or consequential damages arising from your use of our services, including but not limited to:

• Any adverse reactions to prescribed medications
• Failure to achieve desired health outcomes
• Interruption or unavailability of telehealth services
• Unauthorized access to your health information despite our security measures

Our total liability to you for any claim arising from these Terms shall not exceed the amount you paid for your program tier.

9. Privacy

Your use of our services is also governed by our Privacy Policy & HIPAA Notice of Privacy Practices, which is incorporated into these Terms by reference.

10. Governing Law

These Terms & Conditions are governed by the laws of the State of California, without regard to its conflict of law provisions. Any disputes arising under these Terms shall be subject to the exclusive jurisdiction of the courts located in California.

11. Changes to These Terms

We reserve the right to modify these Terms & Conditions at any time. Changes will be posted on this page with an updated effective date. Your continued use of our services after changes are posted constitutes your acceptance of the revised Terms.
